Castiel will be affected because I say so. /ignores angel illness immunity
He started a civil war in Heaven that's resulted in an incredible number of deaths of his own siblings (Crowley said it in canon that the war 'makes Vietnam look like a roller derby') and knows he goes insane and turns on all his friends, wipes out even more siblings, and lets loose a race of ancient evil upon the Earth.
Meanwhile, at SP, there's the whole Sam thing. And the whole Dean mach one dying thing, And the whole getting Crowley involved with the crazy guy that killed him thing. And being stuck around his siblings even more.
Combining with that Castiel canonically says he deserved to die for what happened in the show, he is going to be horribly sick. He may die, not sure; as he hasn't truly experienced the worst parts yet and just knows about them he might manage to survive, just barely, though the in game stuff will drag him down quite a bit so. Undecided.
At the beginning though, before he's out of commission, he'll have his angel powers and will be able to heal people (if even just temporarily) and might be of some use!
